Questionnaire

The Questionnaire activity module allows instructors to create surveys for the students to complete. Questionnaire's can be anonymous or public (allowing the instructor to see individual responses). The questions within the Questionnaire tool can be saved as Public for instructors to use in their course surveys or made Private.

Instructors can add Questionnaires that can be used to survey their students. This can also be used to test basic knowledge without holding the student accountable with a grade.

Surveys for Mid-Quarter Evaluations

A mid-quarter course evaluation can provide valuable information, which allows the instructor to modify their course or teaching methods to improve learning and student satisfaction.

  1. Select Turn editing on.
  2. Click on the Add an activity pull-down list.

  3. Choose the Questionnaire tool.
  4. Under the General section, type the Name (required) of the assignment into the text box.
  5. Type the Summary (required) into the text box.

    Options
    • Enable the use of an open and close date. If enabled, choose the appropriate dates from the pull-down menus.
    • Type sets how many times the student will be allowed to respond to the Questionnaire.
      Note: This can either be once, daily, weekly, monthly, or as many times as the student desires.
    • Respondent type sets how the student is identified. The module can either display the full name or leave the response anonymous.
    • Students can view all responses sets when students can view the overall responses to the Questionnaire.
    • Save/Resume answers sets whether or not to allow students to save their responses in order to resume the Questionnaire later.
    • Submission grade sets the amount of grade that will be given to the students upon completion of the Questionnaire.
      Note: It is possible to give No grade to the questionnaire.
    • Create new creates a new questionnaire that is blank.
    • Copy exisiting will take content from another Questionnaire.If the instructor plans to copy content from another Questionnaire then they will need to click the radio button next to the Questionnaire that the instructor wishes to copy. Copying another Questionnaire will copy all of the same questions into the new Questionnaire that existed in the template Questionnaire.
      Note: If this is the course's first Questionnaire then the only option will be to Create new.
    • Group mode allows the groups that have been created in the course to complete the quiz together.
    • Grouping, which is shown under the advanced options, further combines the groups into larger sets. This allows the collections of groups to all complete the quiz collaboratively.
    • Visible sets whether or not the groups will be able to see one another in the activity.
    • An ID number can be set which allows for easy reference of the assignment in the gradebook.
    • Restrict access allows the instructor to set a condition on the assignment until other assignment(s) are completed:
      • Allow access from and Allow access until determine when students can access the activity via a link on the course page.
        Note: The difference between access from/to dates and availability settings for the activity is that outside the set dates the latter allows students to view the activity description, whereas access from/to dates prevent access completely.
      • Grade condition determines any grade conditions which must be met in order to access the activity.
        Note: Multiple grade conditions may be set if desired. If so, the activity will only allow access when ALL grade conditions are met.
      • Show activity greyed-out, with restriction information / Hide activity entirely determines how the activity displays before conditions have been met.
  6. Choose either Save and return to course or Save and display.



Adding a question

There are a total of 11 options when adding a question to the Questionnaire. There are nine actual question types and two tools.

  1. Click on the name of the Questionnaire activity that was previously added.
  2. Click the tab at the top of the page labeled Questions.
  3. Choose the type of question from the pull-down menu that will be added to the Questionnaire.
  4. Click Add selected question type.
  5. For individual instructions for each question type please refer to the Question types page.

Previewing a Questionnaire

Instructors can preview the Questionnaire that they have created in order to test the functionality of the questions. This can also be used to check the layout of the Questionnaire.

  1. Click on the name of the Questionnaire activity that is going to be previewed.
  2. Click on the tab at the top of the page that is labeled Preview.
  3. Answer the questions as the students are expected to.

Editing a Questionnaire

  1. Click on the name of the Questionnaire activity.
  2. Click Update this Questionnaire at the top of the page.
  3. Edit the Questionnaire as desired. For more information on the settings within the Questionnaire look above.
  4. Click either Save and return to course or Save and display.

Questionnaire permissions

The Questionnaire permissions are established by the system's administrator. The permissions are given to the various roles in order to grant them the ability to perform actions within the activity. The instructor can view these permissions with these steps:

  1. Click on the name of the Questionnaire activity.
  2. In the Settings block, click the Permissions to view all possible capabilities pertaining to the Questionnaire, and which roles have access to each capability. Alternatively, click the Check permissions button specifically view an a list of all enrolled individuals in the course.
  3. Click on the user's name to check and then click Show this user's permissions.
    Note: Type the user's name into the search box in order to refine the display and more easily locate the intended user.
  4. This page lists all of the various actions that are capable within the activity. The list will tell whether or not the user is capable of each action.

Viewing Questionnaire reports

  1. Click on the name of the Questionnaire activity.
  2. Click Logs in the Settings block.
  3. Note: This page displays the activity that has occurred recently on the Questionnaire.
  4. It is possible to filter the report further by making the appropriate choices from the pull-down menus and then clicking Get these logs.

Enabling email notifications

It is possible to receive email notifications that will tell the instructor whenever the Questionnaire is completed.

  1. Click on the name of the Questionnaire activity.
  2. Click the tab at the top of the page that says Advanced settings.
  3. Scroll to the bottom of the page to a box that is labeled Email.
  4. Enter the email address that will receive the notifications.
  5. Click Save settings.
